OLYMPIA — The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department also immediately suspended the powers of persons barred from practicing in other states.

The Department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health professions (eg, dentists, nurses, consultants). Information on disciplinary action against doctors and paramedics can be found at Washington Medical Commission (WMC) website. Information about health care providers is on the agency’s website. Press “Look up a healthcare provider’s license” in the section “How do I?” section of the Department of Health home page ( The site contains information about the status of a medical provider’s license, the expiration date and date of renewal of their authority, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who believe that a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clallam County

In April 2022, the Chiropractic Commission was suspended for at least 12 months. Frank Henry Springob (CH00001149). Springob, without warning, put his hands into Patient A’s pants and began massaging her vagina.

Clark County

April 2022 Nurse Assistant Program and Certified Nurse Assistant. Taylor Nicole Martin (NC60477153) entered into an agreement that suspends Martin’s license for a minimum of five years. Martin stole from several colleagues and vulnerable adults who lived in the nursing home where she worked.

King County

In April 2022, the Minister of Health suspended the registered paramedic’s license for at least 10 years. Jimmy S. Howard (MR60693466). In February 2022, Howard was convicted by the Kitsap Supreme Court of child molestation and immoral contact with minors. He was sentenced to 31 months in prison.

Pierce County

In April 2022, the Minister of Health withdrew the massage therapist’s license without the right to reapply. Danny Alan Calixto (MA60261315). Calixto pleaded guilty and was convicted of two counts of voyeurism for watching two women as they undressed, dressed and showered. He was sentenced to four months in prison and agreed to lose his license.

Snohomish County

In April 2022, the Minister of Health suspended the powers of the Home Care Assistant for at least two years. Mark A. Fabro (HM60678489). Fabro accepted $1,300 from a resident of the nursing home where he worked. He also accepted a check for $100 but did not cash it before the tenant’s family initiated a payment stop on it.

Spokane County

In April 2022, the Minister of Health suspended the license of a Registered Nursing Assistant. Tonya Marie Maki (NA60243131). Mackie admitted to using methamphetamine for four months.

Stevens County

In April 2022, the Minister of Health approved a February order suspending a mental health consultant’s license for two years. Lane Serre (LH00004674). Before being reinstated, Serr must pay a $1,000 fine and complete continuing education requirements. Serre misidentified as a psychologist and violated standards of care by contacting a client’s estranged relative. Serre also managed and claimed partial control of another client’s finances.

Out of state

Montana: In April 2022, the Nursing Commission immediately suspended the Registered Nurse. Katherine S. Fowler (RN61015330). The Montana Board of Nursing found that Fowler committed unprofessional conduct when she reduced the amount of intravenous fluids for an infant without the order of the primary care physician and without communication with the primary care physician.
